BTM Policy & Procedures Agreement Form - 2025

PLEASE NOTE: If the student is under 18 years of age, this form must be completed by their parent/caregiver/guardian.

This form acknowledges that you have received, read and agreed to the terms and conditions outlined in the BTM Policy & Procedures document as emailed to you. (Please note that the Policy and Procedures document has been updated with some changes in 2025)
If you are a part of The SoundCheck Program, this form further acknowledges that you have read and agreed to all conditions and terms outlined in each element's Info Pack.

You acknowledge that these terms may be updated from time to time.

Your agreement and compliance to the terms outlined in that document is necessary in order to receive tuition from BTM. Your tuition maybe be cancelled (and all lessons forfeited) at any time for failure to comply to these terms.

If you have not received the BTM Policy & Procedures document, or if you require a new copy, please request one by contacting BTM.
